All appointments with a GP, urgent or non-urgent, must now be submitted using the online request tool below. If you are required to attend a follow-up appointment after an initial consultation, the GP will arrange this for you. The medical request form submission option is available from 8am until 6:30pm, Monday to Friday and will be reviewed by the GP on the day of submission.
We do encourage you to complete the triage form but, if you are unable to do so or if you would prefer to call us or visit us in person, then this is also fine – our reception team will be able to assist and complete the form on your behalf, but the triage process remains the same.
